CLEAN FLOOR UNDER THE DISH MACHINE,DEBRIS SPECIALLY CORNERS.

LIGHTING: REQUIRED MINIMUM FOOT-CANDLES OF LIGHT PROVIDED, FIXTURES SHIELDED - Comments: Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. DETAIL CLEAN ALL LIGHT SHIELD IN KITCHEN(DEAD INSECTS AND DUST BUILD-UP). ALSO CHANGE DEAD LIGHT BULBS, REAR PREP AREA.

VENTILATION: ROOMS AND EQUIPMENT VENTED AS REQUIRED: PLUMBING: INSTALLED AND MAINTAINED - Comments: Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. REPAIR LEAKING PIPE UNDER THE 3 COMPARTMENT SINK(SOUTH SIDE)OF THE FIRST COMPARTMENT BY THE WALL.ALSO REPAIR LEAKING FAUCETS AT SAID SINK.REPAIR COLD WATER HANDLE AND SAID SINK(UNABLE TO TURN THE HANDLE).

REPLACE BROKEN LIDS FROM OUTSIDE GARBAGE DUMPSTER.LIDS ARE BROKEN FROM THE HINGES, HOWEVER ABLE TO CLOSE.

WE(MANAGER AND I)OBSERVED RESIDENTS SMOKING IN DINING/BREAK ROOM, ROOM WAS FULL OF SMOKING RESIDENTS.OBSERVED RESIDENTS IN LINE WITH CIGARETTES IN THEIR HANDS, AWAITING TO GET IN THE ROOM TO SMOKE.INSTRUCTED TO NOT ALLOW RESIDENTS TO SMOKE INSIDE.
